Our office is approximately 2 blocks north of 23rd street, on the corner of Jenks Avenue and Medical Center Drive, with free parking available.
- Monday – Thursday: 8:30 am - 11:30 am and 12:30 pm - 4:30 pm
- Friday: 8:30 am - 12 pm
At, Florida Sportsmedicine and Orthopaedics we accept Workers’ Compensation, Tricare, Blue Cross Blue Shield, Medicare, and most commercial insurance plans. Please contact your insurance company or you may call our office at (850) 763-0346 to find out if we accept your insurance plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, and MasterCard.
If you have a question related to your bill or insurance, please contact our billing specialists.
If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit. Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.
Dr. Talkington has hospital or surgery center affiliations with Gulf Coast Medical Center and Northwest Florida Surgery Center.
Prescriptions and refills for established patients of Florida Sportsmedicine and Orthopaedics are routinely issued during our regular office hours (see above). You can make your request by calling our office at (805) 763-0346 or emailing us at email@example.com.
Please allow 24-48 hours business hours, excluding holidays and weekends, to process your request. Once we process your refill request, we will call or email you. Please check with your pharmacy before calling the office to check the status of a refill request.
Note that we do NOT call in pain medication after normal business hours. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
